The vacancy has arisen for one Clinical Fellow (ST3+) in Respiratory Medicine, based within the Cardiothoracic Department, at the Freeman Hospital.  

This post is to commence ASAP until 5 August 2025. Candidates must possess full GMC registration with a license to practise at time of application. Experience working within the NHS is desirable.

Main duties of the job

This post is an exciting opportunity to gain experience in Respiratory Medicine, encompassing management of common and advanced respiratory diseases but also aspects of the specialty not readily available elsewhere. In particular, the Freeman Hospital is one of the UK’s leading highly specialised centre for advanced lung disease, pulmonary hypertension, bronchiectasis and sleep medicine services, with an internationally renowned centre for Lung Transplantation. The successful applicant will have a unique opportunity to gain some clinical experience in these respiratory sub-specialties. This post may be suitable for those considering a career in respiratory medicine who want more experience before applying for a Specialist Training post or for those who have completed Specialist Training but want more experience prior to becoming a consultant. To date, all the clinical fellows employed in this post who wanted to pursue respiratory specialist training have gone on to gain a national training number in respiratory medicine.

Detailed job description and main responsibilities

Clinical: 

The successful appointee will an opportunity to work mainly with all the Respiratory consultants based on Ward 29 at the Freeman Hospital which is the primary respiratory ward as well as the ward 38 which is the primary transplant ward based at the Institute of Transplantation at the Freeman Hospital.  

A clear timetable of duties will be provided, but there will be the opportunity to experience some of the other specialist services indicated above, depending on individual interest with the broad case-mix of patients that are referred to attend the Freeman hospital both regionally and nationally with many forms of advanced lung disease. The timetabling of core duties will be determined with Educational Supervisor and Head of Department.

The department also provides a daily respiratory consultation and pleural service to other departments that present a unique opportunity for gaining experience with other regional services, namely Haematology, Bone marrow transplantation, renal and hepatology services, including renal and liver transplant patients, and a regional oncology service.

Administrative: 

Responsibility for in-patient electronic records and discharge summaries for those patients admitted to the ward and communication with Specialist referring teams and General Practitioners regionally and nationally across the United Kingdom as necessary.  Responsibility for updating participant records in Targeted Lung Health Checks bespoke software system.

Other: 

Participation in the weekly Regional ST Teaching Programme is encouraged, plus attending GIM monthly teaching. This post includes on call cover which is for Freeman respiratory and transplant services out of hours on the middle grade rota alongside the respiratory specialist trainees; this is non-resident. This would attract the appropriate banding supplement. On call duties include cover for new admissions and existing respiratory patients on ward 29, as well as respiratory and lung transplant patients based on outlying wards such as ward 25 and 30 in the Cardiothoracic block, and ward 38 based at the Institute of Transplantation. There is support from the Hospital at Night team overnight at Freeman.

Education and Qualifications

Essential criteria
  • Primary Medical Qualification
  • Full GMC registration with a license to practise at time of application
  • Full MRCP(UK) or equivalent

Clinical Skills and Knowledge

Essential criteria
  • Good history & examination skills
  • Able to formulate a working diagnosis
  • Able to order appropriate investigations
  • Image interpretation relevant to practice
  • Knows when to appropriately seek assistance from a senior colleague
  • Evidence of competence in care of Respiratory patients at CMT 2/ST level or equivalent
Desirable criteria
  • NHS experience
  • Emergency level thoracic ultrasound capability (BTS training standard)
  • Capability in undertaking pleural aspiration and chest drain insertion with indirect supervision
  • Evidence of seeing patients with respiratory disease independently in out patient clinics

Maintaining Clinical Competence

  • Written evidence of satisfactory training to date e.g. procedural / clinic logs
