An exciting opportunity has arisen for a Band 5 Nurse to join the Intensive Home Treatment Team.  IHTT provides a citywide service for older people with complex and acute mental health needs, who may be in crisis and can be safely supported in the community.  The aim is to provide robust alternatives to admission, facilitate early transfer from hospital, as well as preventing the breakdown of a caring situation at home.  The focus being on supporting recovery and maximising independence.  It is of significant benefit for older people to have a range of Leeds services working collaboratively towards this shared goal of preventing avoidable admissions and facilitating timely transfer from inpatient settings. The Service operating hours, 8am-9pm, 7 days per week.

We are looking for highly motivated, positive individuals who have a desire to support the wellbeing of older people and a passion for helping those who are experiencing a mental health crisis.

You will work as part of a multi -disciplinary team providing assessment, care planning and care delivery, this will include the provision of highly skilled nursing care, psychological interventions, clinical assessment, developing plans of care and safety plans, risk assessments and carry out home based treatment interventions, assessments/risk management, and promoting recovery and inclusion. You will be involved in managing a team caseload and you will be required to work autonomously. You will have excellent communication skills. You will attend MDT, Best Interest, CPA, discharge meetings for service users. You will also carry out in-reach visits on the wards to service users who will be discharged to IHTT. IHTT operates over a 7-day week, and you will be required to work extended hours (12 hours shifts) over this period.

  • Communication/Relationships - The post holder is expected to demonstrate articulate and effective written and verbal communication skills, with service users and their carers and a wide range of other people/professionals.
  • I.T. - The post holder must maintain and produce detailed contemporaneous and accurate records and reports, in accordance with agreed procedures. IT literacy skills and the ability to work with electronic care records.
  • Knowledge, skills, experience – Must hold a Registered Nurse Qualification for Mental Health with a live NMC registration. Must have experience and knowledge of the problems and difficulties experienced by older people with mental health illness. Must have an understanding of the MHA, MCA and DOLS.  Knowledge / experience of partnership working with other services. Must have a good working knowledge of psychological approaches in mental health. (Has an understanding of the relevant skills required and a commitment to developing skills in psychological thinking and interventions (appropriate to skill level and in line with the serviced integrated care pathways). The post holder will engage in the learning of student nurses, professionals, carers and service users and support/supervise junior staff.
  • Development – The post holder will participate in service development and ensure that own continuing professional development is undertaken.
  • Planning and Organisational Skills - Ability to assess and plan to meet complex needs and manage complex situations
