We also provide some specialist services that are commissioned regionally, such as our medium secure services; and nationally, such as the Cassel Hospital for people with complex and severe personality disorder and our high secure services at Broadmoor Hospital.

 

Job overview

Do you want to be part of a team making Hammersmith & Fulham a great place to grow old?  The Community Independence Service (CIS) provides planned and unplanned nursing and therapy interventions for older residents who have had a recent deterioration in their physical health and is run jointly between West London NHS Trust and CNWL Foundation Trust across the three inner London Boroughs.  

 

We are looking for someone with a passion for community rehabilitation who will be key in supporting the occupational and physiotherapists to deliver effective rehabilitation programmes during a person’s episode of care.  You will develop and maintain relationships that integrate health and social care to support and reduce the need for hospital admission for patients, ensuring a they maintain their ability to engage in daily activities.  This is a great opportunity to consolidate your clinical skills supported through your personal development and appraisal plan

Main duties of the job

Working as a member of the Community Independence Service you will contribute to a person’s episode of rehabilitation to maintain or improve their independence to continue living in their primary residence with as minimal support as possible.

 

You will work under the guidance of health care professionals to assist therapists by carrying out delegated aspects of patient care without direct supervision and reporting back patient’s progress or issues to the relevant therapist.

 

You will undertake specific skilled support work to aid the rehabilitation of patients in the community or the management of patients at home to prevent imminent unnecessary hospital admission.

 

You will be responsible for ordering equipment and department supplies, ensuring that these are received in full and good working order, chasing any outstanding items or actions as required

Qualifications

Essential criteria
  • Attained at least Level 2 Maths and English (equivalent to GCSE grade 9-4 (A*-C)
  • Evidence of continuous development
  • Care certificate
  • Foundation Degree in Health & Social Care/NVQ Level 3 or above or equivalent
Desirable criteria
  • Recognised qualification in relevant subject
  • Attendance on Assistants Training Programme Trusted Assessor trained

Experience

Essential criteria
  • Previous experience in NHS, Healthcare or Social Care
  • Working with patients who have multiple diagnosis including frailty, falls, dementias, neurological conditions
  • Able to work in a team
  • Have knowledge and understanding of the role of Rehabilitation professionals in the community
  • Have a knowledge of conditions that can lead to disability and their impact on individuals
  • Ability to question, clarify, knowledge of own limitations and importance of risk assessment
  • Knowledge of aids and equipment used to promote independence
  • Awareness of confidentiality
  • Clear understanding of role and commitment to role
  • Work in multi-cultural settings
