Job overview

We have an exciting  opportunity for an experienced, senior physiotherapist to join our multidisciplinary integrated Community Neuro/Stroke ESD Team.  This post offers the opportunity to work in the diverse and exciting borough of Hammersmith and Fulham

This role offers an exciting and challenging opportunity to work in our client-focused rehabilitation service.  You will be responsible for assessing and treating clients in their own homes, nursing home placements, and in other community settings. The role also involves providing training and education to clients, their families, carers and other professionals.  

This role comes at an exciting time when we are working with our partners across North West London, looking at specialist provision for clients with complex neurological conditions who are discharged from Level 1 and Level 2 rehab placements into nursing home settings.  Your caseload will mainly be resident in a nursing home environment, with support from a highly skilled wider MDT.

The team’s caseload includes stroke patients referred to our Early Supported Discharge Service, our Specialist Neuro-rehabilitation Outreach Service, delivered in partnership with Imperial College Healthcare NHS Trust, and a wide range of acute and long-term neurological conditions.

Main duties of the job

You will need to be skilled in the assessment and rehabilitation of clients with complex multi-pathologies, including stroke, MS, PD, SCI, MND and brain tumours.
You will need to be a highly skilled communicator, with a passion for service improvement, and and collaberative working across all aspects of the neuro rehab pathway.
You will receive regular supervision as well as support from the Clinical Lead Physiotherapist and you will have the opportunity to develop your own supervision skills through the supervision of junior physiotherapy staff and students.

Essential criteria
  • Diploma/degree in Physiotherapy
  • Current HCPC registration
  • Evidence of up to date CPD portfolio
  • Attendance at specialist short courses or equivalent CPD activities within the specialist areas related to complex neurology.
Desirable criteria
  • Post graduate degree in relevant area
  • Member of a Special Interest Group relevant to this post
  • Member of CSP / ACPIN
