Main duties of the job
The rheumatology team at the University Hospital Southampton NHS Foundation Trust (UHS) are delighted to be able to offer a fantastic opportunity for a customer focused, friendly and enthusiastic individual, as part of our multidisciplinary service.
The Specialist Nurse contributes to the provision of specialist nursing assessment and supervision of patients with rheumatoid arthritis and other inflammatory diseases, practicing within the multi-disciplinary team. They will provide advice, education and support to patients, their carers and other health care professionals. They will practice clinically in agreed care settings according to established protocols or guidelines, and will contribute to the development and smooth running of the service.
The post holder will work to agreed professional, departmental and Trust boundaries as required, to provide patient-centred seamless care. The post holder will be responsible for
- Providing nurse-led clinics for inflammatory arthritis both at UHS and outreach clinics
-Provide patients with advice and emotional support via face to face/virtual clinics, My medical records and rheumatology Advice Line.
- Provide assessment, planning, implementation and monitoring of inflammatory arthritis and programmes of care.
- Liaise with the Rheumatology Medical staff and other Allied Health Professionals
- Providing support and leadership to other staff within the department.

Detailed job description and main responsibilities
Responsibilities
- Provide holistic care to Patient's with inflammatory conditions as part of their clinical pathway in clinic settings including digital platforms.
- Assessment and monitoring of patients conditions and medications providing advice and guidance in line with their individualized care pathway.
- Contribute to the development and delivery of nurse-led initiatives for the benefit of the patient with inflammatory arthritis, in conjunction with medical and nursing/AHP colleagues. To provide regular patient centred nurse led clinics.
- Develop and sustain appropriate multidisciplinary working relationships both within UHS and local ICB's to improve health outcomes and healthcare delivery systems.
- Seek opportunities to develop knowledge, skills and attitudes, including accessing clinical supervision.
Experience and Skill
We are looking for a strong applicant with a passion to provide first class patient centred care in the rheumatology setting.
- Experience in rheumatology is desirable but not essential.
- Experience at a band 5 registered nurse level will be required.
- To be confident in own knowledge base but seek guidance when appropriate.
- Candidate must be confident in working as part of a multidisciplinary team and will be supported to develop independent practice.

Qualifications, knowledge and experience
Essential criteria
- Registered nurse on relevant part of the NMC register
- Degree qualification or equivalent 120 credits at L6
- Post registration qualification within area of speciality.
- Completion of all Band 5 (preceptorship) competencies
- Provide evidence of supporting/ managing a range of learners in practice
- Provide evidence of supporting colleagues to promote effective learning in practice
- Extensive post registration experience within specialist area.
- Knowledge of current issues within the acute health service.
- Understanding of personal accountability.
- Confident of own knowledge base, aware of learning needs and seeks guidance when appropriate.
- Able to critically review and communicate clinical information.
Desirable criteria
- Registered on MSc Pathway – completion of HTPA Competencies
- Act in the role of Practice Assessor or supporting the progression of learners / supporting challenges as needed
