Job overview

Ealing Intermediate Care service has an exciting new role within the Senior Management Team (SMT) for an enthusiastic and motivated candidate with management experience. You will have experience of working with multiple providers to provide professional and strategic leadership in line with agreed service objectives. 

Within this role you will work as part of the SMT within ECP and LNWHT with a focus on embedding the D2A model of care across Ealing in collaboration with NWL sector peers.

The Integrated Discharge Hub based within Ealing hospital facilitates and tracks discharges across the D2A pathways and supports flow across the North West London (NWL) system.  The service is led by West London NHS and includes multiple providers; London North West University NHS Trust, Ealing CCG and Ealing local Authority. It builds on the set up as part of managing COVID-19 identified in government guidance issued in March & August 2020

Main duties of the job

Lead on the development and embedding of shared care planning and trusted assessments through joined up working and closer integration of services across acute, community, social care, mental health services, and third sector provision of an integrated and co-ordinated efficient and clinically cost effective care wrapped around a patient’s needs to improve wellbeing and health outcomes

In collaboration with partners, the hub lead will implement an integrated daily patient list. This will be a live data set for all agencies to work from and include those suitable for discharge, the number and percentage of people on the list who have left the hospital, and reason of delay for those unable to be discharged in a timely way.

Develop and maintain shared governance and performance across providers and partners

To provide strategic oversight of the discharge to assess process ensuring that there are no delays to discharge and that a “home first” approach is adopted.

To act as intermediator between neighbouring boroughs (CCGs, community, acute and LA)

To provide strategic oversight to WLT, LNHWT, Ealing CCG, Ealing LA and neighbouring boroughs.

Develop internal capability to support partner functions

Work within a multi-agency environment at a senior level to embed joint working and collaborative approach to D2A 

To work with peers within respective organisations to ensure discharge timescales as laid out in national guidance are adhered to.
