Job overview

As part of expansion of specialised services, an exciting opportunity has arisen for a new dedicated ACP service within the thriving Gastroenterology and severe intestinal failure (SIF) service at University Hospital Coventry. We are looking to appoint  motivated staff to train as advanced clinical practitioners to help support delivery of care for complex patients on our new SIF Ambulatory Assessment Unit (SIF AAU) in addition to supporting management of scheduled Gastroenterology day-case admissions.

Through specialised commissioning of intestinal failure services in the UK to help support safe delivery of care to patients with intestinal failure, UHCW was recently designated as one of two dedicated severe intestinal failure (SIF) centres for the West Midlands.  As a SIF centre, we help to support medical and surgical management of patients from across the West Midlands with complex intestinal failure.  This includes inpatient transfer for management of select complex patients, outpatient review and follow-up, and ambulatory care and treatment of long-term complex patients.

Successful candidates will undergo training to support patients attending for review on the SIF AAU and scheduled Gastroenterology Day-case admissions area.

Main duties of the job

The successful applicant will be supported to develop into a high profile, accessible and responsive leader with managerial and clinical responsibility for the delivery of Advanced Clinical Practice in Gastroenterology, Hepatology, and Intestinal Failure (IF). Able to make informed decisions within their own speciality and scope of professional practice, involving patients and public to ensure agreed standards, targets and objectives are met.

They will become an experienced healthcare professional qualified to Masters Level, who will act as a clinical leader within the Gastroenterology inpatient service. Once qualified they will autonomously assess patients attending for review on the SIF AAU and scheduled Gastroenterology Day-case admissions.  This will include initial history taking, clinical assessment, diagnosis, and initiation of treatment plan with close involvement of the relevant sub-speciality services as appropriate (e.g., Nutrition Team, Hepatology, Gastroenterology). They will demonstrate safe, senior clinical decision-making and expert care, for patients providing appropriate clinical management plans and prescription of any medication required.  They will also be expected to develop competence in relevant interventional procedures such as diagnostic and therapeutic paracentesis.

Detailed job description and main responsibilities

The successful applicant will be supported to develop critical thinking in the clinical decision-making process. They will work collaboratively with the Intestinal Failure, Hepatology, and Gastroenterology team to meet the needs of patients, and will play a key role in service delivery and adherence to policy and procedures.  Once qualified, they will be instrumental in guiding junior medical and nursing staff in relation to relevant protocols and procedures.

·         Co-ordinate the care of Gastroenterology and IF patients throughout their pathway with the multidisciplinary team and clinical services to ensure the effective management of this group of patients.

·         Accountable for own actions in accordance with the professional Code of Conduct and regulatory guidance. Will contribute to corporate objectives, acting within local, Trust and statutory guidelines and policies at all times.

·         Lead and deliver care across a variety of clinical settings such as outpatient, inpatient, virtual clinic, ward rounds, etc.

·         Deliver clinical care according to national and local guidelines ensuring delivery of evidence-based care and working towards the Trust vision of providing a World Class Service. 

·         Assess and prioritise patients within the service to maintain quality care.

·         Undertake an initial assessment to determine differential diagnosis, utilising advanced critical thinking and decision-making skills, deciding when necessary to refer to senior clinical colleagues.

·         Act autonomously to provide expert clinical care which contributes to the patients’ treatment plan to ensure that optimum physical, psychological, and social care needs are met.

·         Request and interpret relevant investigations to facilitate the development of the patient’s management plan.
