We are seeking a highly motivated individual with excellent interpersonal and communication skills to work in our Community Heart Failure Service. The successful candidate must be committed to supporting patients with a diagnosis of Heart Failure delivering focused specialist nurse led interventions as part of MPFT Long-Term Conditions Service.

As previously stated we will consider recruitment of a suitably skilled nursing candidate, who has qualified in physical health assessment skills and can demonstrate appropriate experience of working in cardiac or heart failure care clinical settings to commence at Band 6 and would be supported to undertake the prescribing module to then step up to Band 7 Specialist Nurse remit.

Essential elements of the role include assessing, planning and the implementation of holistic packages of care to patients and their carer’s who have a diagnosis of Heart Failure.

As an autonomous non-medical prescriber you will provide community and domiciliary specialist care providing timely and appropriate optimisation of evidence based therapies. The main aim of the role is to provide an intense level of care  and treatment intervention optimising the use of latest cardiac therapies, to support the individual to manage their condition, preventing unnecessary admissions to hospital and to improve the patients overall quality of live up to and including provision of specialist palliative care directive.

The successful candidate will need to be a NMC registered nurse who is a non-medical prescriber and has the relevant specialist post- graduate (Masters) qualification, and/or professionally recognised clinical courses in Heart Failure Management; Or significant equivalent experience with considerable evidence of professional development. You will demonstrate strong clinical reasoning and prioritisation skills, and be able to work effectively, both independently and as a member of a team and a commitment to professional development, contributing to a culture of continuous improvement

  • You must be a qualified nursing practitioner with an active and current NMC registration
  • Hold a demonstrable background of cardiac focused clinical experience,
  • Ideally hold qualifications in heart failure patient clinical care
  • Hours are 9am-5pm Monday to Friday. There is also some flexibility around start and finish times.
  • You are required to hold a driving licence and have access to a car.
  • Small team with administrative support.

  • Registration with NMC
  • V300 nurse prescribing or completed within first 12 months of obtaining the position
  • Mentorship qualification (998-997)
  • Relevant specialist post graduate (Masters) qualification and/or professionally recognised clinical course(s) in specialist area or significant equivalent experience with considerable evidence of professional development
  • Ability to interpret specialised cardiac reports

  • Supporting service users, families and carers in dealing with heart failure and its associated complications
  • Experience of negotiation and conflict management in clinical situations
  • Experience of leading clinical audit in specialism
  • Supervision of more junior members of a team

Skills, Knowledge & Abilities

  • Evidence of expert/advanced clinical knowledge relevant to post
  • Understanding of Information Governance Data Protection, NMC standards and Safeguarding Vulnerable Adults
  • Knowledge of relevant NICE guidance
  • Understanding of wider NHS agenda and its application to the local context
  • Excellent written and verbal communication skills, including the ability to effectively communicate information of a sensitive or distressing nature
  • IT skills

  • Experience managing a caseload with clients with a diagnosis of heart failure
  • Knowledge of health and social care legislation, policies, objectives and how they apply to day to day practice
