Our service provides Early Supported Discharge (ESD) for both Stroke and Acquired Brain Injury (ABI) patients across Somerset and specialist inpatient and ongoing community stroke rehabilitation.

Our service has received excellent feedback from patients, carers and commissioners, as well as high levels of staff satisfaction and has been awarded an innovation award by the Community Hospitals Innovation Team.  We strive to use current best practice and do so by regularly reviewing our service and adapting to provide patients with the best possible outcomes using evidence based practice.

We now have opportunities for dynamic, motivated and experienced individuals to join our Stroke and ABI ESD service.  This innovative service delivers intensive rehabilitation within patients’ homes using state-of-the-art mobile technology, ensuring patients are kept at the centre of all decisions, goals, intervention and discharge plans.

The team consists of Occupational Therapists, Physiotherapists, Nurses, Speech and Language Therapists, Psychologists and Rehabilitation Assistants jointly providing seamless services to patients within their homes and along the whole stroke pathway.

To support this service, we require additional Rehabilitation Assistants to work as part of the team and contribute to the efficient countywide service.

Main duties of the job

  • To work as an integral member of the Health Care /Rehabilitation Service providing effective support to Nurses, Occupational Therapists, Physiotherapists, and any other team members.
  • To work with patients undertaking therapy/rehabilitation programmes to support and encourage them to achieve their optimum level of independence and quality of life, as directed by the team of therapists/nurse’s.
  • To work with patients in a range of locations, including hospital departments such as wards, rehabilitation unit, outpatients, and the community with overall supervision from rehabilitation professionals.
  • To manage own time effectively demonstrating flexibility to meet the service requirements across the county
  • Community Hospital wards and rehabilitation areas, in patient’s own homes with and without Team professionals
  • Travel throughout the team Locality may be involved on a regular basis

Qualifications and Training

Essential criteria
  • Previous Adult/elderly rehabilitation, nursing, care worker experiences experience working in adult rehabilitation/ care experience
  • Post holder will be required to achieve NVQ 2/3 or equivalent in care/rehabilitation
Desirable criteria
  • NVQ Level 2/3 or equivilent in relevent field.
  • Experience of working with stroke, neurological, ABI patients

Knowledge & Experience

Essential criteria
  • Understanding and experience of lone working
  • Experience of working as part of a team
Desirable criteria
  • Previous NHS experience
