Band 6 Community Psychiatric Nurse / Occupational Therapist

37.5 hours per week, 7 days a week, 8am - 8pm.

We are seeking an enthusiastic and motivated Band 6 Mental Health Nurse to join our  MHSOP Intensive Home Treatment Team within the City. We provide assessment, treatment and support for service users of any age with a diagnosis of a dementia or people over the age of 65 with a mental health condition within an 8 week time frame. 

The team provides specialist assessment, therapy & treatment, working to an individually tailored care plan to adopt a flexible, patient-centred approach enabling the older person with mental health difficulties to engage in services available to them and to lead as independent a life as possible.

Delivered in people’s own homes within the community, the service makes every effort to prevent unnecessary admission to hospital and facilitates the opportunity for people to return home following a hospital admission or period in residential care.

There will be an expectation that the team will work closely with current Community Mental Health Teams (CMHT) in the locality and also with the CRHT’s.

There will be an expectation  that the Band 6 Clinician will be the duty worker for the team.

As a Band 6 CPN/OT your main duties will be

To provide a defined level of specialist assessment and access to interventions for a diverse patient group and their families under the care programme approach, who present with significant mental health conditions and people with a dementia diagnosis.

To provide intensive support to patients within their own community setting.

To demonstrate active clinical and managerial leadership skills within the team, providing education and supervision to junior staff members and other professionals.

Swydd ddisgrifiad a phrif gyfrifoldebau manwl

You will provide Community Mental Health nursing/OT input into the service. You will also provide specialist assessments and provide support and supervision to junior team members.

You will be an experienced Registered Mental Health Nurse or Occupational Therapist and have current NMC/AHP registration. You will have proven post qualification experience and have experience of working with this client group and of multi-disciplinary working. You will have a sound knowledge of risk assessment and be able to demonstrate sound evidence-based practice.

A full UK driving license and vehicle for business use is required for this post; however reasonable adjustments will be made for disabled individuals in line with the Equality Act 2010.

This post is eligible for a Golden Hello payment of £2,750 for new starters who are external to the Trust.  This is payable in 3 instalments over a 2 year period.  This payment is only eligible to new starters who do not already hold a contract of employment with the Trust.  This post also includes paid enhancements for unsocial hours working during night, weekends and bank holidays.
