Job overview

This is an opportunity to join the diabetes specialist nurse team at the Elsie Bertram Diabetes Centre at the Norfolk and Norwich University Hospital. The aim of the department is to provide patients with a high standard of care with a view of improving both health and quality of life. The Diabetes specialist nurses work across three different teams supporting patients seen in the diabetes centre, inpatients and out in the community. This role will predominantly support patients in the community either undertaking nurse led clinics in GP surgeries or in their homes however, there will be opportunities to work with the other teams. The role would also involve giving group education to patients but also teaching to practice nurse’s and GP’s.

Main duties of the job

Working autonomously undertaking nurse led clinics within a variety of settings including GP surgeries, patients own homes, care homes and telephone clinics. Undertaking one to one education but also group education to improve patient’s knowledge to assist them to make informed decisions about their diabetes care. Working closely with practice nurses and GP’s to identify gaps in knowledge and deliver training where required. Supporting patients with lifestyle choices and dietary requirements to make better decisions to improve their health. This role can allow for career furthering opportunities including but not restricted to independent nurse prescribing which will allow you to change and prescribe medications with the aim to improve patients’ diabetes and overall health.

Detailed job description and main responsibilities

  • Implement Trust and Directorate evidence-based procedures, policies, protocols and guidelines.
  • Contribute to local policy making within the agreed Trust strategy. 
  • Act as an ambassador for the Trust in professional and trust business settings. 
  • Develop policies and procedures for nursing practice for patients with diabetes. 
     
    To belong to diabetes interest groups at local and national levels, undertake presentations and disseminate information gained to unit staff. 
  • Manage own caseload of patients. 
  • Provide nurse-led clinics in specialist areas. 
  • Act as mentor to local Diabetes Peer Supporters. 
  • Ensure that communication links between the Diabetes Centre, Inpatient Wards and Primary Care are strengthened. 
  • Provide clinical and managerial leadership and support to all nursing and clerical staff within the diabetes team 
  • Assist with managing staff performance and development through a process of appraisal, which should include a cycle of constructive performance review and the establishment of a relevant personal development plan.
  • Document incidents on Datix and escalate as required.

Qualifications

Essential criteria
  • Registered Nurse on the appropriate part of the NMC register
  • ENB 928 or equivalent
  • ENB 998 or equivalent
  • Continuous up-date to KSF evidence portfolio
  • Proof of continuous update in specialist areas.
Desirable criteria
  • Independent Nurse Prescribing
  • Have, or be working towards a relevant Degree qualification

Experience

Essential criteria
  • Significant experience at Band 5
  • Some experience in a position relevant to the speciality

Skills

  • Ability to provide high quality patient centred care
