Clinical Lead for Multiple Long Term Conditions (CBC)

Clinical and Care Professional Leads support south east London-wide and borough programmes of work, therefore, there is an expectation that you currently work within south east London, and for borough based roles, within that borough.

Clinical and Care Professional Leadership (CCPL) roles have been established in SEL ICB since April 2022 and have been a valuable source of clinical leadership. 

Within the Community-based Care (CBC) directorate, the role of CCPL for multiple Long Term Conditions has an important remit, providing advice and support on complex system-wide transformation for patients living with multiple LTCs in SE London.

The immediate priorities for the postholder will be to:

•    Support the new Multi-Morbidity Model of Care (MMMoC) Programme,
including direct clinical advice and support to the pilot PCNs and their efforts to spread and scale their Integrated Neighbourhood Team models to the rest of their LCPs. 
•    Act as a clinical champion, more generally, for the further development of a consistent, personalised model of care, operated by INTs, in line with the key recommendations of the Fuller Report and the SEL Framework of LTC Care. 
•    Support the agreed priorities of the CBC directorate and LTC Team, in line with the SEL Joint Forward Plan, including the priority areas of diabetes, CVD, respiratory and mental health and well-being (in the context of co-morbidity with a physical LTC).  

The role will strongly link with the other CCPL roles within the CBC directorate and LTC-related CCPL roles at place and the postholder will be expected to make links with other LTC and Primary Care stakeholders at Programme and Place level. 

In agreement with the postholder, a more detailed workplan will be developed after appointment.

Qualifications and Experience

  • Qualified care and/or clinical professional i.e. NMC, HCPC, social work register, GMC etc. Educated to Masters level or equivalent level of experience of working at a senior level in specialist area.
  • Experience of transformational/innovative service delivery, productivity and/or transformation policy implementation.
  • Previous experience of being part of peer networks and collaborative working
