Main duties of the job

 
 

The main responsibilities of the post holder are: 

  • To provide a central point of access to the BHR IDH ensuring day-to-day administrative duties are carried out enabling clinicians to increase their case management time
  • To provide high standard, smooth, efficient and organised administrative support liaising with the IDH clinical lead and other staff.
  • To assist the IDH team by liaising with other services, organisations, partners and agencies with all aspects of the IDH function
  • To act as a point of contact for within NELFT services, acute partners, other agencies and service users to deal appropriately with enquiries and escalate as required.
  • To monitor the BHR IDH e mail inbox for any new referrals, email correspondence, queries and liaise with IDH staff promptly
  • To accurately input activity data onto the IDH spreadsheet and databases. 
  • Use RIO efficiently and update any relevant information as guided by the therapist. 
  • To support the IDH team in the monitoring and delivery of directorate KPIs and contracted performance targets.
  • To support the IDH team with the production of accurate, accessible performance reports for the performance group and directorate teams

Barking & Dagenham, Havering & Redbridge Integrated Discharge Hub (BHR IDH) is  based in Goodmayes Hospital to provide high quality service to all patients who are 18 + and clinically safe to be discharged from hospital. The expectation of the service is to reduce the length of stay for people in acute care, to improve people’s outcomes following a period of rehabilitation and recovery and minimise the need for long-term care at the end of a person’s rehabilitation.

  • NVQ level 2 or equivalent standard of literacy and numeracy
