Job overview
The GP extensivist will be working closely with the ACE consultants and the ACE MDT, reviewing new and follow up patients and providing the medical part of the CGA, identifying and managing frailty, rationalising medications, managing multi-morbidity, arranging rapid diagnostics and co-coordinating further MDT input to address the patients functional and psycho-social needs. They will also collaborate with community and primary care teams regarding further management. There will be ample one-to-one teaching from the ACE consultants who work on the unit on a consultant of the day model with direct and indirect supervision and support as appropriate.
The ACE clinic runs Monday-Friday 9am-5pm. The successful applicant(s) will have the option of doing from one day a week on a Tuesday.
Main duties of the job
The successful applicant(s) will join a dynamic, enthusiastic and friendly ACE team and elderly care department that provide high quality sub-specialty services including stroke care, Parkinson’s disease, orthogeriatrics, falls and intermediate care. There may be opportunities to utilise and develop a workplace based assessment tool relevant to GP practice/training as well as combined competencies for the RCGP and RCP geriatrics curricula.
