Job overview

Welwyn and Hatfield Integrated Care Team are winners of Hertfordshire Community Trust Leading Lights awards for Demonstrating Fantastic Patient Care and are looking to recruit band 5 Nurses to our award-winning team. Hertfordshire Community Trust has a Good CQC rating acquired in May 2020 and we are working towards achieving an Outstanding rating.

We deliver high quality nursing care across the Welwyn and Hatfield locality to prevent admission to hospital, facilitate discharge, deliver planned nursing intervention and manage long term conditions.  

The successful candidate would be supported to develop skills and join a team who are committed to demonstrating the trust values who care about each other as well as their patients.

We work collaboratively with our partnership organisations across East and North ICB, Hertfordshire Foundation Trust and Hertfordshire County Council.

Main duties of the job

Welwyn and Hatfield Integrated Community Team is an expanding and evolving service. We provide a wide range of nursing led care to people within their own homes. As a nurse in our team, you will be supported to develop and broaden your skill set. To achieve this, we facilitate our staff to access an extensive training and education programme, as well a range of personal and professional development opportunities including the PgDip Specialist Community Nursing course. For our newly qualified staff, we have set up a forum to facilitate a monthly meeting of peer reflection and training, designed to support those wanting to join the community soon after completion of their nurse training, as well as a trust wide preceptorship programme to develop your CPD portfolio and skills.

Our service operates 8am – 10pm, 365 days a year. We can offer flexible working patterns and pride ourselves on working with our staff to ensure their work schedule supports a healthy work life balance. We are happy to consider full or part time applicants.

A full valid driving licence and access to a car to use regularly for business purposes is essential (unless you have a disability as defined by the Equality Act 2010).

Detailed job description and main responsibilities

Our nurses are committed to teamwork, have good communication and decision making skills and consistently work collaboratively with other professionals within HCT and beyond. We aim to provide a high standard of patient care using an evidence-based framework.

Successful applicants will be responsible for assessment, planning, implementation, and evaluation of individualised nursing care to patients living in the community. They will need to support the Nursing Sisters of the team to manage the caseload and support junior staff.

A commitment to health promotion teaching and training is essential, to optimise our patients ability to self-manage their health needs and engage them in maintaining their wellbeing.
