A rare opportunity is currently available to work in the School Nursing Service term as a School and Community Health Facilitator, either full time or part time.

In Gloucestershire the School and Community Health Facilitator will work within the School Nursing Service providing school entry vision screening, National Child Measurement Programme and advice and support for children and families either one to one or in facilitated groups in the community.

Main duties of the job

We are seeking a School and Community Health Facilitator

  • to undertake School Entry Screening programmes,
  • to provide healthy lifestyle interventions for children and their families
  • to support the School Nursing Service delivering packages of care to families on an individual basis and in groups
  • to facilitate discussions with groups of parents about overcoming common health issues in children
  • to facilitate health promotion groups for parents

Ideally you will have experience of working with children.  Good IT skills are essential together with a commitment to developing innovative ways of delivering services.  You will receive training and work through competencies and have further opportunities to develop professionally.

Detailed job description and main responsibilities

·      Undertake screening programmes for vision and hearing 5-19 years age group – planning, managing and prioritising their own workload.

·      Undertake the monitoring for NCMP Programme for Reception and Year 6 children.

·      Liaise with parents, health professionals and Public Health Nurse Team (PHNT) with screening results.

·      Assist in the care of children and young people before and after vaccination when required.

·      Facilitate health promotion and health education sessions for parents known as School Nursing Hubs.

·      Make every contact count by promoting healthy lifestyles at every contact.

·      Refer to PHNT where there are concerns regarding a more complex need.

·      Deliver health education and health promotion sessions and take part in events/fayres which focus healthy lifestyles

·      Promote engagement in services and pro-actively signpost to other services as appropriate.

·      Keep accurate and contemporaneous records of all client contacts including child health records and inputting of screening data.

·      Participate in audit, survey, research and development activities.
