An exciting opportunity has arisen to join the MSK Therapies Team within East Sussex as an MSK Care Coordinator working with the wider Musculoskeletal rehabilitation service.

This is a substantive post and a new role in our service. As such we are looking for someone with patient facing experience and exceptional communication and organisational skills. You will be a key instigator in the role’s development and will need to be able to work flexibly within an evolving remit within the MSK team and wider Trust Rehabilitation Services.

You will be working 1:1 with patients, enabling them to engage with our service. You will be working with our teams to make our services more accessible to patients especially those at risk of health inequalities.

 

You will be part of the patient engagement team, developing our service in response to patient feedback. The role requires excellent communication, organisational and ICT skills, as well as the ability to remain calm, professional and enjoy working as part of a fast-paced multi-disciplinary team.

 

You will need to be able to communicate confidently and empathetically with patients who may have additional social needs and link with clinical teams, administrative teams, the VCSE and Social Prescribing Services.

 

The successful candidate will need to have an understanding of personalised care and an enthusiasm to learn. We will support you to develop as a health coach and motivational interviewer and to complete the National Care Coordinator development framework with the support of our teams.

Qualifications

  • Good level of general education including A levels and GCSE or equivalents including functional skills level 2
  • Evidence of continued professional development to NVQ level 4 in relevant subject or equivalent level of knowledge and experience
  • To have completed or working towards a portfolio of CPD underpinned by the competencies outlined in Competency framework for care co-ordinators

  • ECDL or alternative IT qualification

Experience

  • Experience of computerised record systems and data storage / retrieval / analysis
  • More than 3 years’ experience of working in health, social care or information and advice, in direct contact with people, families and carers in a paid or voluntary capacity

  • NHS administration experience in a similar post.

  • Good communication skills to communicate effectively with customers, careers, specialist services and colleagues verbally and in writing
  • Good keyboard and IT skills including knowledge of Microsoft Office – Word, Excel and Outlook
  • Ability to identify, coordinate and organise health and social care support within required timescales using negotiation and motivational skill to promote health
