The Dorset ICS are seeking experienced clinical leaders to support with progressing our vision for Home First delivery. These posts, employed by Dorset Healthcare on behalf of the ICS, will be required to work across the system to achieve the strategic ambitions of Home First, and Discharge to Assess.

This is a fixed term role and for those with current NHS service, the role will be offered on a secondment basis only.  Please obtain permission to be released on a secondment before submitting an application.

The post holders will be part of a team providing a leadership role to locality clusters across Dorset. The Cluster Lead roles being recruited will have a focus on East Place area, enabling and facilitating timely discharge across pathways, providing an interface with community teams and sharing information regarding previous community involvement with acute partners, with a requirement to provide an element of inreach support to our acute partners University Hospitals Dorset and Dorset County Hospital, while working in partnership with Local Authority colleagues.

These roles are currently fixed term until 31st March 2025 with the intention that this period is used to evaluate and inform the transition to and adoption of a new Transfer of Care Hub model.

To undertake the role successfully there will be a need to utilise clinical experience of out of hospital care, and leadership skills to maximise the opportunity for admission avoidance, flow, assessment following discharge, and successful completion of patient journeys’ through discharge pathways using a strength based approach.

Employment in this post requires an Enhanced Disclosure and Barring Service (DBS) check, which the Trust will cover the cost of.  Applicants who subscribe to the DBS update service are able to present a valid DBS certificate instead of requiring a new check.

  • Relevant full Masters degree or demonstrable extensive equivalent experience in the relevant specialty.
