Job overview

Post holder required to ensure  placement of patients -right bed right patient .

Work closely with all disciplines to ensure patient flow

Main duties of the job

Ensure that all patients are placed appropriately, working with all disciplines to facilitate discharge and ensure patient flow through the emergency pathway

Detailed job description and main responsibilities

Correct patient placement 

work with all wards to identify ,plan and enable discharges

work with emergency department to place patients correctly based upon clinical priority 

work with infection prevention and control team to ensure all patients are placed safely reducing the risk of infection transmission

work with partners in discharge services to facilitate discharges 

provide relevant correct and meaningful data daily to wider distribution list
