The role of Co-occurring Conditions Community Care co-ordinator is to work in partnership with service users who are experiencing both mental health and substance use issues, developing servicer user care plans and recovery plans to enable them to lead fulfilling lives. The post holder will demonstrate an understanding of clinical interventions relevant to the locality teams protocols to include assessments, harm minimisation, motivational interviewing, psycho-education, risk management, relevant aspects of social inclusion, recovery, care plans and discharge planning, working closely with the local drug and alcohol services, carers, families and partner agencies to facilitate this
You will have a degree or relevant professional registration as a Nurse, Occupational Therapist or Social worker or equivalent substantial working experience with the designated client group, You will be an active member of a dynamic, highly motivated, skilled and creative team
You will be working in a supportive environment that values your supervision and professional development
You will have a voice and role to play in service development.
Main duties of the job
The ideal candidate will work at all times to promote the safety and the wellbeing of service users and their families/carers. The post holder will assess, plan and implement care, and provide specialist harm minimisation and reduction of substance use advice and carry out specialist clinical skills and procedures. They will provide clinical supervision to staff and students. The post holder will ensure that the locality NICE interventions and ethos are adhered to.
The ideal candidate will promote a positive culture of integrated working with substance misuse services for service users, carers and family.
The ideal candidate will have a strong team work ethic, sense of accountability, accompanied by an ability to work under pressure and with a desire for innovation. We are able to support agile working for staff, using all appropriate I.T equipment and support, and have a secure area for staff bicycles. The city provides a good opportunity to be active at work, with staff being able to cycle to local appointments.
You will be involved in the delivery of care-coordination to a specified caseload operating under the CPA framework, a daily duty service, ongoing risk management, care planning, support around benefits and housing, carers’ assessments, physical health care and a wide variety of therapeutic interventions for which you can access training and supervision.

1. To undertake assessment of individuals and their families/carers accessing the service with complex health presentations, including those service users presenting with higher levels of risk.
2. To develop alongside the service user, their care plan.
3. To offer specialist professional assessment and advice where required.
4. To assess and manage on going risks as identified during the assessment, ensuring that this is done in collaboration with the service user and carers.
5. To regularly review risk factors and make changed to the management of them as necessary.
6. To work using assertive outreach approaches to engage the service user and their families.
7. To contribute to the maintenance and development of the Trust nursing strategy.
8. To ensure that clinical practice is evidence based and consistent with relevant NICE guidelines and Trust policies and procedures.
9. To provide supervision of junior staff and trainees where appropriate.
10. To act within the professional code of conduct at all times.
Person specification
Education/Qualifications
Essential criteria
- Registered Nurse with current registration [mental health/LD/Adult], appropriate to the job role OR, Registered Social Worker OR, Registered OT, OR Educated to a degree level relevant to the work area or equivalent substantial working experience with the designated client group
- Experience of working in a clinical drug and alcohol service
- Evidence of continuous professional development to include evidence based brief interventions; motivational interviewing and harm minimisation training
Desirable criteria
- Teaching/cert/psychology degree level
Experience
Essential criteria
- Significant post qualifying experience in working with those presenting with serious mental illness and substance use
- Experience of working alongside people with complex health problems in the designated care group
- Experience of providing a range of clinical interventions to people in the designated care group with a variety of health problems
- Experience of conducting clinical assessments including risk assessments
- Experience and knowledge of co-occurring conditions substance use.
- Previous experience of working in the community
