As part of our dynamic multi-disciplinary team, you'll have the opportunity to lead the delivery of high-quality clinical care while promoting independence and enhancing patient experiences. With the chance to interface regularly with inpatient colleagues, you'll play a crucial role in ensuring safe patient discharges and facilitating early transitions to community-based care. Your role will also involve leading the assessment of inpatients, exploring opportunities for early and safe discharge into the community under the care of ICRT. Familiarity with the biopsychosocial model and expertise in medication management, including safe administration, are integral aspects of this role.
