The post is based at our main central hub, Wentworth Avenue in Slough, which is a very accessible part of town, but which gives quick access to the main areas as well the neighbouring localities.
The ideal candidate will be someone who has worked in an acute health care setting at Band 6 and feels ready for a next level challenge. They need to be motivated to work in a high demand and clinically exciting service. The ideal candidate will be someone who truly values the opportunities that the mental health professionals have in assisting people going through difficult times in their lives. They will be passionate about helping people with SMI getting back on their recovery journeys following relapses. We pride ourselves of our accessibility and about our high ranking on consecutive national benchmarking network reports.
Rotas are always completed 2 months in advance to give individuals time to plan their lives.
The team is very supportive with many very experienced practitioners from different clinical backgrounds.
Prif ddyletswyddau'r swydd
- Taking or discussing referrals to the service.
- Carrying out reviews for patients referred to the crisis resolution home treatment team.
- Involved in the delivery of care of the patients taken under the care of CRHTT
- Attendance to referral meetings
- Take part in complaints and serious incident investigations
- Liaising with other parts of the mental health service
- Assessing and supporting patients both via home visits and phone/video calls
- Participating in the QMIS process for service improvement
- Supervising junior staff and student

- Evidence of relevant post registration training and development

- Considerable experience in the assessment and management of clinical risk

Previous Experience

- Experience of working in CRHTT
- Experience working in an Emergency Department environment

Knowledge/skills

- Advanced understanding of the mental health needs of adults
- Specialist knowledge of person centred care and its application
- Specialist knowledge of psycho-social interventions and their appropriate use
- Working knowledge of appropriate assessment tools
- Working knowledge of the policies and guidelines pertinent to the planning and delivery of care to adults with mental health needs
- Recovery focused practitioner able to apply recovery principles to adults and older people
- Clear understanding of psychotropic medication and the monitoring of effects and side effects

- Knowledge of practical application and use psychological therapies
- Knowledge of service improvement models and its application
- Awareness of the impact of the health economies on service planning and delivery
